Anna (Milla Jovovich) becomes an innocent witness to the latest murder of a serial killer who terrorizes the city. But on the run from the crime scene and the killer, she falls and strikes herself unconscious. When she wakes up in the hospital, she is diagnosed with prosopagnosia, or the inability to recognize faces, and is unable to recognize her own family, boyfriend or face in the mirror. As the only one who has seen the killer, she can now not trust anyone.
